Mesothelioma Trials

Mesothelioma is a relatively rare form of cancer linked to chronic asbestos exposure. There has been a sharp decline in asbestos exposure since the 1970s when asbestos use was halted for most applications. However, the incubation period for mesothelioma from initial exposure to asbestos to the onset of the disease varies from 20 to 50 years. As such, deaths related to this disease have risen recently and are expected to continue rising until the year 2020. This anticipated increase in incidence of mesothelioma has spurred considerable interest in clinical trials to develop better treatments for the deadly disease.

Mesothelioma Clinical Trials
Until recently, clinical trials for mesothelioma were difficult to conduct. The disease had a rapid onset and progression, leading to a relatively short survival period in which to conduct clinical trials. Also, there was a lack of basic understanding about the clinical biology of the disease and how the disease changed during progression.
Specific clinical measures were difficult to obtain in order to demonstrate the response to treatment. Recently, as part of the Alimta clinical trial, researchers developed new metrics that included factors other than just pleural thickening or abnormal breathing capability. A number of new clinical trials are now underway, many of them using these new measurement techniques. Hopefully these trials will lead to ways to stop or reverse the progression of mesothelioma.
Mesothelioma Trial Phases
Clinical trials for evaluating mesothelioma treatments are broken into four parts or phases. These Phases are described below (Source FDA).

Phase IResearchers evaluate a drug or treatment in a small group of people/patients (20 to 80) for the first time to evaluate a drug’s safety, determine the appropriate dosage range and identify side effects.
Phase IIResearchers treat a larger group of people/patients to determine if the drug treatment is effective and safe for the dosage levels determined in Phase I.
Phase IIIResearchers administer the drug or treatment to a large group of people/patients to confirm the effectiveness of the drug and evaluate the drug’s side effects. In this phase, the drug under evaluation will be compared to commonly used treatments. Additionally, researchers use Phase III trials to determine the proper protocol for the use of the drug when it is released for routine treatment.
Phase IVResearchers perform follow-up studies from Phase III, primarily to obtain information that can be used when marketing the drug, such as drug risks, benefits and optimum use.

The stadiums mesothelioma

The Mesothelioma is a form of cancer that are difficult to diagnose. The symptoms are chronologically. This creates a lot of confusion in some doctors, because certain symptoms associated with other diseases. Good synchronized diagnosis can be very difficult, because the rest period between infection and onset of the signs. This could be up to 40 years to fully express itself, the damage that it has caused. If the symptoms occur, what cancer is already at an advanced state. At that time, the treatment for the patient enough. Treatment options are depending on the stage of cancer. Mesothelioma pleural mesothelioma, in four steps in peritoneal mesothelioma is not staged.

Phase I shows the presence of mesothelioma either on the right or the left pleura or stomach. Step II of the attacks in the chest wall or in the esophagus, heart, or pleura on both sides. During Phase III, in the lymph nodes can be found and can continue in the chest. This is also the point at which the mesothelioma penetrated his stomach mucosa into the abdominal cavity. The phases II through IV is rated as the advanced cancer. Stage IV is as advanced mesothelioma and, in most cases, it is an opening / closing of the surgery, because it is no longer useful. The cancer mesothelioma is difficult, in the not too. Originally, tumors can be seen out in the membranes surrounding the lungs and abdominal cavity. She then metastases in the bodies nearby.

The spacing of the steps through tomodensitométries, MRI tests and other mechanisms, such as the use of X-rays. As these tools, the physician can see the extent to which the cancer has spread. Through these measures, the doctor will be able to maintain this that the level and type of treatment, at what stage. Once the stage, and the treatment has been determined, the doctor while he speaks with the patient and his family. At the moment, where the mesothelioma is diagnosed, most of the treatment is the management of pain and control systems, drug therapy and psychological methods to help patients to better manage the impact of pain.

Treatments such as chemotherapy, radiation therapy or surgery are not sufficient without other medication. But if the traditional methods are no longer an option, the experimental methods will be considered. Clinical studies on the people to the effectiveness of new drugs are for the people. When a patient is accepted, will be participating in a clinical trial, the patient receives, the scientific analysis of the efficiency of research.

In total, the treatment should always be dependent on the localization of the tumor, at what stage the cancer is, its spread and the way mesothelioma cancer metastases. Factors such as age and health status of the patient can be used as during the treatment.

The management and treatment of cancers of mesothelioma is a very difficult. The patient must be physically and emotionally prepared for the consequences, a cancer of mesothelioma. Regardless of the Medical Association of prognosis, the patient must be able to prepare for what will follow.

A study shows the improvement pleural mesothelioma patients who cisplatin more raltitrexed

Mesothelioma pleural mesothelioma patients treated with cisplatin and raltitrexed may survive longer than patients who cisplatin alone, according to a study in Europe and Canada (J Clin Oncol. 2005 Oct 1, 23 (28): 6881 - 9). Raltitrexed (Tomudex), with a capacity of the cancer cells to multiply and make the deoxyribonucleic acid or DNA. The cisplatin is a medicine from conventional chemotherapy.

Two one hundred fifty patients with pleural mesothelioma participated in a clinical trial to determine the safety and effectiveness of raltitrexed. Hundred Twenty-four patients received cisplatin, and the rest has more raltitrexed cisplatin. Most were men with an average age of 58 who do not have the additional treatment with chemotherapy.

Raltitrexed The study was a phase III clinical tests, as proposed by the European Organization for Research and Treatment of Cancer. Phase III comparison a new therapy against cancer with a standard treatment, and requires a large number of participants. It follows on from a clinical study of the phase II, which deals with the safety of a new treatment and the way the administration and a clinical phase I trial of the efficacy tests drugs on a small number of patients.

Increase the survival rate among patients with raltitrexed The researchers have investigated the survival, the progress of pleural mesothelioma, security, the tumor response rate, the patients and the quality of life. They have discovered, what follows:

“About six percent of patients with pleural mesothelioma in the cisplatin group raltitrexed survived at least one year, compared to 40% of the cisplatin group.

“The average survival was of 11.4 months for patients in the cisplatin / raltitrexed group, but only 8.8 months for patients in the group with cisplatin.

“Twenty-four percent of patients receiving cisplatin / raltitrexed had a positive response against the pleural mesothelioma, compared to 14% of patients receiving cisplatin.

“The quality of life in general was roughly in the group of cisplatin compared to the cisplatin group raltitrexed.

A small number of patients, both in the cisplatin cisplatin and raltitrexed groups developed lower white blood cell count (neutropenia). Other side effects included fatigue, nausea and vomiting, often in the group of combination drugs.

Raltitrexed is relative to the Pleural Mesothelioma Drug raltitrexed Alimta ® is in the class of so-called antifolates chemotherapy. It blocks the production of thymidylate synthase, back, which decreases its ability mesothelioma cell, the DNA, the genetic material of the base, in the reproduction of cells. It is in Europe for the treatment of advanced colorectal cancer.

Raltitrexed is closely connected with pemetrexed (Alimta ®), another antifolate. Currently pemetrexed / cisplatin is the only combination therapy of chemotherapy, that the U.S. Food and Drug Administration has in the treatment of pleural mesothelioma.
