Mesothelioma Survivors - the lung or pleural cavity

The Patient :

My aunt has been diagnosed with mesothelioma though it is not clear whether this is a few cancerous cells or a full tumour. We believe it to be fatal. Could you explain where this is in terms of the lung or pleural cavity. Also what should we be pushing for in terms of treatment. She has had one lot of radiotherapy. I think she will take an 8hr chemotherapy session shortly. There is mention of a clinical trial but have heard that such trials are more likely to cause discomfort than be of help.

Her husband has oesophogeal cancer and we are unsure of the survival rate/life expectancy there too. He has had chemotherapy and is now undergoing radiotherapy. The tumour has not reduced in size so far. He has had a stent put in and can eat foods if mashed.

Any help or suggestions would be very much apprieciated.

I am sorry to hear about the cancer diagnoses.

80 percent of mesothelioma is pleural in origin and is typically associated with asbestos exposure. The median survival of patients with mesothelioma is between 6 and 18 months, and is not significantly affected by currently available therapeutic interventions.

Chemotherapy and radiotherapy are currently the recommended treatments of choice.

Experimental approaches include intracavitary therapy (directly injecting compounds into the pleural space) and gene therapy. Unfortunately, there is minimal success with these treatments, but they may be worth discussing with your personal physician.

Mortality of esophageal cancer depends on the stage. The mortality rates for stages I-IV are 60, 31, 20, and 4 percent respectively.

Mesothelioma: The Silent Killer

On September 11, 2001, 2,602 people died in the terrorist attacks on the World Trade Center. Thousands more may die a slow and painful death caused by exposure to asbestos released in the collapse of the towers. Immediately after the attacks over 100,000 people were exposed to dangerous levels of asbestos and over 670,000 New Yorkers may still be at risk for developing environmental illness.

;Mesothelioma is cancer caused by asbestos exposure. The symptoms of mesothelioma can take 10 to 50 years to appear, but once the disease is diagnosed it progresses rapidly and most patients have less than a year to live. Asbestos levels after the 9/11 attacks were so extreme that symptoms have appeared as soon as two years after exposure.

The danger could have been minimized
The Environmental Protection Agency (EPA) repeatedly tried to issue warnings about the threat of airborne contaminants. These warnings would have given emergency workers and others the chance to protect themselves from airborne contaminants. The warnings were removed from the EPA press releases by the White House Council on Environmental Quality. In April, 2007, the U.S. Court of Appeals issued a decision supporting the Council’s actions.

First responder first to die
Emergency responder Deborah Reeve developed symptoms of asbestos related illness in 2003 and was diagnosed with mesothelioma in 2004. While she was save the lives of others during the aftermath of 9/11 she was unknowingly being exposed to massive amounts of asbestos which, leading to her own death in 2006. In the words of her husband David, “(Deborah) got killed on September 11, and she didn’t die until March 15, 2006. She got killed and didn’t know.”

The evidence was hiding in a shirt
Yehuda Kaploun worked at ground zero for about 48 hours immediately after the collapse. He saved the shirt he was wearing to honor the fallen of 9/11. In April, 2006, the New York Post reported that when a portion of that shirt was analyzed, it was found to contain 93,000 times the amount of chrysotile asbestos normally found in American cities.

The World Trade Center Cough
You may not have heard about mesothelioma in connection with 9/11, but you probably have heard of “the World Trade Center cough.” In April, 2007, the Centers for Disease Control (CDC) reported that 62% of those caught in the dust cloud, and 46% of those not caught in the cloud but living or working in the area now suffer from respiratory problems. The Mount Sinai Medical Center’s World Trade Center health study found that 85% of its 70,000 participants suffer from respiratory problems. First responders have been suffering respiratory problems since immediately after the attacks.

Who may be affected?
First responders are among those known to be adversely affected, but they are not the only ones. Anyone who was in the area during and after the collapse of the towers may have been exposed to dangerous levels of asbestos. The contamination lasted for weeks or months after the collapse. Even as tests were showing enormous amounts of asbestos in the air, officials were urging the public to return to the downtown area. People who lived and worked in the area, children going to school in the area, and anyone who happened to pass through, may have been affected. Because the disease can take up to 50 years after exposure to develop, it will be decades before we really know how many people were really killed by the events of 9/11 and the government’s cover-up of the contamination.

How Is Mesothelioma Diagnosed?

Diagnosing mesothelioma is often difficult, because the symptoms are similar to those of a number of other conditions. Diagnosis begins with a review of the patient’s medical history, including any history of asbestos exposure. A complete physical examination may be performed, including x-rays of the chest or abdomen and lung function tests. A CT (or CAT) scan or an MRI may also be useful. A CT scan is a series of detailed pictures of areas inside the body created by a computer linked to an x-ray machine. In an MRI, a powerful magnet linked to a computer is used to make detailed pictures of areas inside the body. These pictures are viewed on a monitor and can also be printed.

A biopsy is needed to confirm a diagnosis of mesothelioma. In a biopsy, a surgeon or a medical oncologist (a doctor who specializes in diagnosing and treating cancer) removes a sample of tissue for examination under a microscope by a pathologist. A biopsy may be done in different ways, depending on where the abnormal area is located. If the cancer is in the chest, the doctor may perform a thoracoscopy. In this procedure, the doctor makes a small cut through the chest wall and puts a thin, lighted tube called a thoracoscope into the chest between two ribs. Thoracoscopy allows the doctor to look inside the chest and obtain tissue samples. If the cancer is in the abdomen, the doctor may perform a peritoneoscopy. To obtain tissue for examination, the doctor makes a small opening in the abdomen and inserts a special instrument called a peritoneoscope into the abdominal cavity. If these procedures do not yield enough tissue, more extensive diagnostic surgery may be necessary.

If the diagnosis is mesothelioma, the doctor will want to learn the stage (or extent) of the disease. Staging involves more tests in a careful attempt to find out whether the cancer has spread and, if so, to which parts of the body. Knowing the stage of the disease helps the doctor plan treatment.

Mesothelioma is described as localized if the cancer is found only on the membrane surface where it originated. It is classified as advanced if it has spread beyond the original membrane surface to other parts of the body, such as the lymph nodes, lungs, chest wall, or abdominal organs.

The Main Types Of Malignant Mesothelioma Cancer

What is mesothelioma? This name refers to a very rare type of cancer that has some thousands of cases on all over the world. This type on cancer is caused by inhalation of Asbestos. Asbestos are some materials used in construction manufacturers.

There are four types of mesothelioma cancer, three types of them are very dangerous and are very hard to treat, there three types are Pleural Mesothelioma “Tumors develop in lung lining”, the second dangerous type is Peritoneal Mesothelioma “Tumors develop on Peritoneum, the outer membrane of the internal organs in the abdominal cavity, the third dangerous type is Pericardial Mesothelioma “Tumors develop in the sac that contain the heart”. Pleural Mesothelioma is the most common type of that cancer, the most patients of that cancer are diagnosed as victims. The main reason for this disease is the inhalation of Asbestos, these Asbestos was used at construction manufacturers especially the cement manufacturing companies.

Symptoms and Treatment of Mesothelioma Cancer

Mesothelioma cancer is often caused by exposure to asbestos. People who have received asbestos exposure of as little as one or two months to very low doses are at risk of this. A person exposed to asbestos could develop mesothelioma after 50 years of exposure. The 2 common types are pleural mesothelioma (lung cancer) and peritoneal mesothelioma (abdomen organs).

There are no immediate symptoms of mesothelioma cancer. The symptoms may appear in 30 - 50 years after exposure to asbestos. The following symptoms may be noticed after several years of exposure.

Symptoms of pleural mesothelioma cancer: shortness of breath and pain in the chest.

Symptoms of peritoneal mesothelioma cancer: abdominal pain and weight loss, blood clotting abnormalities, anemia and fever.

Approximately 20% of pleural effusions remain without an established aetiology after evaluation. Thoracoscopy has a very high sensitivity for the diagnosis of both benign and malignant diseases and greatly increases the diagnostic yield for pleural effusion. We sought to evaluate the diagnostic yield and safety of medical thoracoscopy at this institution.

The records of all patients undergoing medical thoracoscopy for the evaluation of undiagnosed pleural effusion between 1990 and 1996 were reviewed. The procedure was performed under local anaesthesia with sedation using a Stortz rigid thoracoscope. Fifty-eight patients had thoracoscopy, most having had two (range: 1-6) non-diagnostic pleural aspirations and biopsies of the pleura. Nineteen patients were found to have mesothelioma and nine metastatic malignancy. Three patients were considered likely to have tuberculous pleural disease, six had asbestos related benign pleural fibrosis and three post-cardiotomy syndrome.

There was one chylous effusion of uncertain aetiology, one posttraumatic and two other benign effusions, both of which resolved without clear aetiology. On seven occasions the pleural space could not be adequately accessed, but none of these patients had prior computerized tomography (CT) or ultrasound of the pleural space. There were five false negative diagnoses of malignancy, but no false positives.

The diagnostic sensitivity for pleural malignancy was 85% and specificity 100%. There were no major complications, but four patients had late tumour seeding at the thoracoscopy site. Medical thoracoscopy is a safe procedure with a high diagnostic yield. Pre-operative evaluation of the pleural collection using ultrasound or CT increases the likelihood of successful access to the pleural space and may increase diagnostic yield.

Introduction to Malignant Mesothelioma
Malignant mesothelioma affects the lining or membranes of certain large cavities in the body. These cavities, called the serous cavities, house certain major organs in the body including the heart, lungs, abdomen and others. The membranes that surround these cavities are called the serous membranes. They serve to protect these major organs from the friction and abrasion that occur as the organs move against each other during typical daily functioning, such as breathing and heart beating. The serous membranes derive from specialized cells called mesothelial cells. These cells form to create the mesothelium, which is the major tissue layer of the serous membranes. Cancer that occurs in the mesothelium tissue is called mesothelioma.

Malignant mesothelioma takes on three forms:
Peritoneal mesothelioma, in the peritoneum or membrane surrounding the abdomen. A rare form of peritoneal mesothelioma can also affect the male testicles, because the lining around the scrotum is an extension of the peritoneum.
Pericardial mesothelioma, in the perdicardium or lining surrounding the heart.
Pleural mesothelioma, which is the most common form of malignant mesothelioma and affects the lining of the lung cavity.
All mesotheliomas are now considered to be malignant, or cancerous. They start as a localized tumor and spread rapidly to include most of the surrounding tissues and organs. Originally, fibrous cists and other non-cancerous growths that appeared in the serous membranes were thought to be mesotheliomas and were termed “benign fibrous mesothelioma” and “multicystic mesothelioma.” These conditions are now known not to originate in the mesothelial cells, but other cells within the serous membranes. These conditions are typically localized, non-cancerous and do not spread rapidly.
Etiology of Malignant Mesothelioma
Most cases of malignant mesothelioma are associated with a previous exposure to fibrous minerals, of which asbestos is the most important. There is now indisputable evidence of the role of these fibers in the development of this disease. This has been demonstrated in animal experiments and many other experiments. Not everyone who is exposed to asbestos fibers has developed malignant mesothelioma, suggesting that other factors may also be involved.
Although malignant mesothelioma is rare, it is a serious, deadly disease. It is often in the advanced stages when it is detected, so the outlook for the patients is not as good as it is for the cancers that are detected earlier. The average survival time after detection is 1 to 2 years. This survival time also depends on the type of mesothelioma.
